Does how IHOP still give free birthday meals ??? How do I fill out one of those cards?

I looked at a few websites, but none of them say if you need a card or not. Please see the attached link. It gives you a ton of places to go for free stuff on your Birthday!! HAPPY BIRTHDAY!
